Sharing Incidents and Near Misses

We want to bring safety even more into focus at this year's conference by drawing on your experiences. We invite you to share an incident or near miss and how you handled it.

A refiner at the 2003 Coker seminar had the courage to talk about their $7 Million fire and give an evaluation of the incident, root cause analysis and lessons learned.  You can bet that all of us took those cautions home with us.

We encourage you to step forward from the FCC world and share your experiences so we can all be safer in the CatCracker.

Here's from another refiner: "I would be glad to share an incident that happened in the last 24 months. I would like those that participate to consider not only 'incidents' but 'near misses'. Near misses are situations that got identified before an incident happened but would have led to an incident with just a slightly different set of circumstances or with a little more time. I'm interested in hearing about mechanical failures from inadequate inspection or wrong material installed, situations where process conditions changed (temp, press, rate’s) that caused an incident over time, small fires, environmental releases.  These are as much value to us as the recap on a big incident."
